The Boys has continuously provided shocks that are unmatched by other TV shows over the years. We have already seen some raucous moments in Season 4, such as Rob Benedict’s Splinter and a deliciously disastrous rehearsal for Vought on Ice. However, the third episode, which you can stream on Amazon Prime, also featured a big reveal involving Sister Sage, one of Season 4’s new superheroes. Like that gory lobotomy tool, the unsettling lengths she went to quiet her brain have lingered in my memory, and I am left wondering: Was this an isolated incident or did it happen often?
Sister Sage’s Lobotomy Reveal Was One of The More Horrifying Things I’ve Seen on The Boys
As Sister Sage, who has the superpower of being the wisest person in the globe, Susan Heyward has joined the cast of The Boys Season 4. She was recruited to The Seven in “We’ll Keep the Red Flag Flying Here,” a role that clearly unnerved her but was required to appease home lander (Antony Starr). Following her observation of the milk-loving supe’s instability when he arbitrarily killed another person, Sister Sage made the delightfully simple decision to veg out.
We are talking about the fundamentals of Say Yes to the Dress and Bloomin’ Onion eating. When she expressed interest in The Deep (Chace Crawford), it became evident that something was wrong. Sure enough, the camera moved down to reveal a bloody instrument, implying that she had committed self-lobotomy.
The idea that Sister Sage would need to undergo a full lobotomy and stab herself in the eye—perhaps behind her eyelid—is downright terrifying. To receive some relief, in her ear? —into her brain. Is that possible the darkest thing we have seen on The Boys so far, out of all the awful stuff we have seen?
The idea that the smartest person on the planet would have to purposefully harm their brains to enjoy some guilty pleasure TV, greasy takeout, and maybe a pointless hookup also makes me feel bad about humanity (or myself) in general. Are any of us slightly damaged brains? Which brings me to my main query.

Was This a One-Time Lobotomy, Or Is This Her Typical Way of Winding Down?
Since this is the first proof of Sister Sage’s lobotomizing practices that we have, we are unsure of the long-term consequences. Did she know it would be impossible to cooperate with home lander and that she would have to give up her full gift to stay one of his henchmen? Will her brain be healed by her superpowers, allowing her to temporarily escape the actual world?
I lean towards believing the latter. I get the impression that this was not the first time she did this from the way she carelessly left the lobotomy tool on the table. Given her nature, I assume she understands how to play with her brain and how long the consequences will linger. If that is the case, is this something Sister Sage engages in regularly, or only in extreme circumstances — like after watching an innocent woman get lasered in the head?
The Boys’ Eric Kripke Gave Us the Lowdown on Rob Benedict’s Raunchiest Scene, And the Hilarious Nickname Supernatural’s Misha Collins Gave It
Here are some spoilers for those who have not watched the three-episode Amazon Prime launch of The Boys Season 4, as well as possibly mild spoilers for those who have.
When it comes to its frequently shocking subject matter that divides critics, The Boys has never been one to back down, and Season 4 took a bold and direct approach. At least that is what Rob Benedict, the Supernatural vet,’s brief new hire did during his big role in the second episode, “Life Among the Septics.” And showrunner Eric Kripke revealed to us the most astounding—as well as the most repulsive—detail of that shocking salad-tossing incident.
To set the scene correctly, Frenchie and Kimiko entered a scene that preceded Splinter’s insane naked fight, which Kripke also discussed with Cinema Blend. In this scene, Benedict’s character was performing what I will now refer to as a conga line, but it was more of an oral pleasurable dance. It goes without saying that not every actor would want to be in that role, but according to Kripke, the actor was committed right away, even after learning what was involved. To quote him:
Yes, look, I told everyone to stop when I found out Rob was considering the position. I must give him a call. I must make it very clear to him what he is getting into. He is a companion. I am unable to use any of this to sabotage him. When I told him, “Here’s every scene your character will be doing if you get this role,” over the phone, he agreed. It was hilarious, in his opinion.
To compare the finalized version that Eric Kripke had to tell him about to the specific role description that Rob Benedict was initially given that piqued his attention, that would be great to know. In the end, it did not appear to matter because the erstwhile God portrayer looked quite content to film for a week straight while eating his own butt out and just wearing a prosthetic penis. Hopefully, in time, if there is not a special award for that, that will change.
Oh, and when I mentioned that he was “eating his own butt,” I was not exactly being cynical or exaggerating—the effects team carried that notion out to its natural conclusion, as Kripke informed me:
Indeed, the object in the sauna is a photo-realistic latex mold of his actual butt that, well, you get the idea. [Giggles] It is a spectacular visual effects achievement.
As if Rob Benedict would accept an invitation from The Boys to participate in an arc in which he would slurp up some random butt mold. These are experts in their fields. Consequently, Kripke concurred with me that the scene’s foley and sound design elements are equally as potent as their visual elements.

The Boys has one of the most consistent ensemble casts on television, and its protagonist and major antagonist are portrayed by individuals who are simultaneously incredibly funny and very terrifying. Of course, I am referring to Karl “Billy Butcher” Urban and Antony “Homelander” Starr, whose frightening personas were entirely turned upside down in The Boys’ amazing Season 2 blooper reel. We now have confirmation for people who were wondering if Starr experienced any problems after consuming so much milk.
As hilarious as the program itself, The Boys Season 2 blooper film features a range of odd stumbles, line errors, camera bumps, and more erratic flubbings and features nearly every important actor in the cast. (No Noir, but that is only because he is flawless in most aspects.) Enjoy the video below while sipping a nice, cool, refreshing, wet, thick, and creamy glass of milk!
To be honest, I think my favorite part of the blooper reel would have been filming the scene where Karl Urban’s character Butcher angrily tries to shoot his gun at Stormfront. However, there was an errant leaf stuck to it, which made the scene look about 33% less badass but also 33% more funny. If only that person had not been so tolerant of lactose. Not precisely what that specific setting required.

Laughing a lot, I am sure it must have been hilarious to laugh at some of the takes, which resulted in the milk being ingested needlessly, in addition to having to drink milk repeatedly during takes. And to have to add a hint of nostalgic, pervy yearning to that performance as well. I would have asked, “Can we make this chocolate milk and then change the color of it in post?” as soon as I saw that scene. Then it could have caused blech faces like the one above to arise.
Unfortunately, there were no awkward moments where Antony Starr’s Homelander freaked out about his fake clone or lost it when he pushed his son out of the house room. It is probably advisable to avoid having any “blooper” footage when children are seen falling from heights. (Although the scene was not filmed on a roof.
However, when Jack Quaid transforms into Pennywise the Clown from IT, kids really should not be around.
